RESIDENTIAL AND LONG-TERM CARE CENTRES
CENTRE D'HÉBERGEMENT ET SOINS DE LONGUE DURÉE (C.H.S.L.D.)

The elderly and adults who have become temporarily or permanently dependant for physical or psychosocial reasons, are offered a specific range of services enabling them to remain longer in their home or benefit from a residential setting adapted to their degree of dependency.
 
The C.L.S.C., as the gateway to the system, assesses the need for the following: home care, psychosocial consultation, domestic help, nursing care, equipment loan, specialized services, palliative care, respite and sitting service and support for the natural caregivers.
 
The dependent elderly and adults who can no longer remain in their homes should get in touch with their C.L.S.C. who will carry out the assessment and make the referral to the most appropriate resource. The needs of hospitalized people will be assessed by the hospital social worker. If transfer to a long term care facility appears necessary, the request will be forwarded to the regional orientation and admissions committee co-ordinated by the C.H.S.L.D. of Hull.
 
People who reside in these facilities must make a financial contribution based on their ability to pay. Community organizations also offer home and community assistance and support for the natural caregivers.
